FREQUENTLY ASKED QUESTIONS

Welcome!! Thank you for choosing Synergy Physical Therapy and Yoga, Inc. for your physical therapy needs. We look forward to working with you in achieving your goals of a happier, healthier, and more functional you!

CAN I SEE A PHYSICAL THERAPIST WITHOUT A SCRIPT OR REFERRAL?

In Colorado you can see a physical therapist without a referral.  Medicare requires a doctor’s script to continue treatment after the first visit.

WHAT SHOULD I WEAR?

Wear comfortable clothing and footwear that allow you to move easily, and allows the physical therapist to view the area we’re working on. For example: T-shirt or tank top and shorts. We do provide shorts and gowns if necessary for your treatment.

WHAT SHOULD I EXPECT ON MY FIRST APPOINTMENT AND FOLLOW UPS?

First Appointment: Your initial evaluation will be 40 minutes. Please bring information that may be helpful to your therapist, such as your prescription from your doctor, protocols/directions from surgeon if you are recovering from surgery.
Your therapist will review your medical history and symptoms, conduct a thorough examination to determine what factors may be contributing to your pain, discuss your goals and expectations for physical therapy, and develop a plan of care.
We ask that you be on time for this appointment so that you get the full benefit of your first treatment. If you prefer to fill paperwork out upon arrival, please plan to arrive 15 minutes early. We recommend parents accompany patients under the age of 18 for the first treatment.
Follow Up Visits: Appointments will be 40-45 minutes in length, and include assessment of your progress, followed by treatment. Treatment may include manual interventions, as well as appropriate modifications to your exercise program.

Testimonials

Physical Therapy Session

"I would highly recommend Sarah Evans to anyone in the Evergreen area seeking physical therapy. She provides thoughtful and professional care, through the use of deep tissue work and stretches/exercises. She helped manage my pain and gave me the tools necessary to help facilitate my recovery at home. Her caring and personable demeanor made going to PT enjoyable."

Heather Brown, Evergreen, CO

"I have nerve damage in my neck and shoulder that causes pain constantly. To make my chronic situation manageable I've been going weekly to Christine and sometimes Sarah for soft tissue work, exercise and dry needling. I get excellent care and a drastic reduction in pain as well as good advice for management and stress reduction. Also, Georgie knows how to handle insurance issues and scheduling without fuss. These folks really know what they're doing and I look forward to my visits."

David Bright, Kittridge, CO

"I highly recommend Synergy Physical Therapy in Evergreen. Over eight years I have received knowledgeable and caring relief from Christine and Sarah whenever my chronic condition flares. I also leave with sheets of exercises to use to forestall a recurrence. All the staff are so cheerful and helpful."

Susan Hoffmann, Evergreen, CO

“I started going to Synergy Physical Therapy due to a frozen shoulder caused by a skiing accident. My surgeon gave me a choice of going to PT or having surgery. I have been very pleased with my shoulder improvements due to the physical therapy. The pain has been substantially reduced, and I have regained most of the mobility in the shoulder and will not need surgery. The therapists at Synergy Physical are active in continuing education as both students and instructors. They are excellent at understanding the issues that I had, and making sure that I received the most advanced treatment to ensure a speedy recovery.”
